Player Waiver

WAIVER, RELEASE, AND INDEMNITY AGREEMENT

In consideration of my child’s acceptance to participate in Fun Go Volleyball programs and my permission for them to participate in all related activities, I, as the parent or legal guardian of the child(ren) named below, hereby RELEASE, WAIVE, and FOREVER DISCHARGE:

The staff, coaches, and volunteers of Fun Go Volleyball

Bill Hogarth S.S and any other school or location where the programs may take place

Any other associated organizations, sanctioning bodies, sponsors, and all their respective agents, officials, employees, contractors, representatives, successors, and assigns

from any and all claims, demands, damages, costs, expenses, actions, or causes of action, whether in law or equity, arising from any injury, death, loss, or damage to my child(ren)’s person or property, regardless of how it was caused. This release applies to incidents occurring before, during, or after my child(ren)’s participation in the program, whether as a participant, competitor, spectator, or in any other capacity, even if such incidents were caused or contributed to by the negligence of the aforementioned parties.

I acknowledge that participating in sports activities associated with Fun Go Volleyball involves inherent risks. I understand that by allowing my child to participate, I am freely accepting and fully assuming all associated risks, dangers, and hazards, including the possibility of personal injury, death, or property damage.

Furthermore, I agree to HOLD HARMLESS and INDEMNIFY all the above-mentioned parties from any liability or claims that may arise in connection with my child(ren)’s participation in the program.

I also acknowledge and agree that Fun Go Volleyball may capture and use photographs and videos of program events, including participants, for promotional purposes.

By registering my child(ren) for the summer camp and other programs, I confirm that I have read, understood, and agreed to the terms outlined above. I also warrant that my child(ren) is/are physically fit and able to participate in the program.
